If you are ever asked for information via an e-mail asking you to click on a link, it will be fraudulent. ALWAYS REPORT SUCH ATTEMPTS TO THE ORGANISATION CONCERNED.
If the request is genuine, you will be asked to log into the institution's web site in the normal way (NB do not use any helpful clues from the e-mail) and then provide the information. If in doubt, e-mail the organisation and ask whether they actually require the information, or come to this site and ask here.
